Augustana Mercy Care Center is seeking a Resident Care Coordinator to lead and direct its Transitional Care services. This full-time opening in the Health Care Center provides the opportunity to be a part of the management team, direct a new TCU program, and work with residents and direct nursing staff. The successful candidate will be familiar with long-term care, Medicare reimbursement rules, the MN case mix system and take pride in the delivery of excellent service.
